Healthcare in Portugal for Golden Visa holders

A Portuguese residence permit lets you live where public healthcare is open to registered residents and private care costs much less than in the US. What it is worth depends on how much time you spend in Portugal, and on when you put private cover in place.

  • ~€1,850Average monthly price of a private care home in Portugal, early 2026

Can a Golden Visa holder use Portugal’s public health service?

Yes, if they live in Portugal. A foreign national with a valid residence permit and a Portuguese tax number can register with their local SNS health unit and receive a user number, which gives access to the national health service.¹

Since June 2022, the SNS charges no user fee for primary care consultations, prescribed tests or referred hospital consultations; a fee remains only for emergency visits without a referral.² The SNS is organised around where you live: registration is with a local health centre, so a holder who keeps only the minimum stay should not count on it and will rely on private care or travel cover during visits.

What does private healthcare cost?

Portugal has a large private sector, with hospital groups in Lisbon, Porto and the Algarve and many English-speaking doctors. Without insurance, a specialist consultation typically costs about €60 to €150, more in some Lisbon hospitals.³

Private care in Portugal Indicative price
Specialist consultation, uninsured About €60 to €150³
Health insurance, plan for over-55s From about €50 a month per person at 57⁴
Health insurance, couple aged 60 to 65 Roughly €800 to €2,100 a year, depending on cover⁴

Is there an age limit for private health insurance?

Yes, and it matters for planning. Portuguese insurers set a maximum age to join: standard family plans typically accept new members up to their early or mid sixties, while plans built for older clients accept them up to about 70 or 75. Some plans also limit how long late joiners stay covered: joining a standard plan after 55 can mean cover ends at 65.⁵

Waiting periods of 60 to 90 days apply to new members before some cover starts. The practical conclusion is simple: cover is easier and cheaper to put in place earlier, which is one reason families secure residence before they need it.

What does long-term care cost in Portugal?

A private care home in Portugal costs about €1,850 a month on average, from about €900 for residents who need little help to about €3,500 for high dependency, and somewhat more in Lisbon.⁶ In the United States, the median cost of assisted living is $6,200 a month, and a private nursing-home room $10,798.⁷

United States Portugal
Assisted living or care home Median $6,200 a month⁷ Average about €1,850 a month⁶
Highest level of care Private nursing-home room, median $10,798 a month⁷ Up to about €3,500 a month⁶

Figures are not converted between dollars and euros, and US assisted living and a Portuguese care home are not identical services.

Does Medicare cover care in Portugal?

No, in general. Medicare generally does not pay for care received outside the United States; some supplemental plans cover emergencies abroad.⁸ An American who spends long periods in Portugal in retirement therefore budgets for Portuguese care separately, usually through the SNS once registered, private insurance, or both.

How does the value change over a lifetime?

The permit costs the same whenever it is secured; what it is worth for healthcare depends on how much time you spend in Portugal, and when you put private cover in place. For many families it is an option held in reserve in their forties and fifties, and a place to spend long periods in retirement and late life.

Stage Typical age What the residence permit does for healthcare
Building 40s Secures the right to live in Portugal early, while private cover is easier to obtain
Transitioning 50s Gives a European base before Medicare age, and time to put private cover in place before entry-age limits
Retired 60s and 70s Allows long stays in Portugal, with SNS registration once living there and private care at Portuguese prices
Late life 80s and over Allows a holder to live in Portugal, where private long-term care costs much less than in the US

Holding the permit on a week or two a year does not change US healthcare costs by itself. The saving comes when a holder actually spends time in Portugal, and that is a decision the permit leaves open. For the wider picture for American families, see Portugal’s Golden Visa for Americans.

Is my family covered too?

Each family member included in the application holds their own residence permit, so each can register with the SNS on the same terms. Spouses, children, and dependent parents and parents-in-law can all be included on one donation; see who your family can include and the projects.
